Position Title: Data Analyst
Reports to: Executive Director

Mission: To provide free quality medical care and referral services with dignity and compassion to the uninsured of our community.

Vision: To provide for the medical needs of the uninsured while connecting them to existing community resources. To share Christ’s love and hope with patients and volunteers alike.
Job Description: This role will manage documentation, pull reports, and manage data reporting. In addition this role will have understanding of the clinic and addressing the overall health and needs of the patients served by the clinic.

Duties and Responsibilities:

Define organizational values and assist in the creation of a mission that will support the transformational process

Analyze the effect that the Shalom chronic care program has on our patients’ health, wellness, socioeconomic situation, and satisfaction with care

Facilitate population health approach to understanding and addressing the overall health and needs of the patients served by the clinic

Provide reports, high risk patient profiles, and charting feedback to Lupe and Lindy’s Care Team on a monthly basis

Communicate with other departments and providers concerning patient outcomes and clinic compliance measures to encourage and aid in quality improvement

Survey patients on clinic accessibility and care satisfaction

Collect, evaluate, and report data for clinical and organizational outcomes and measures through learning how to navigate i2i Tracks Population Health Software and eClinicalWorks

Assist in leading Quality Improvement and subsequent initiatives, using data to drive practice and processes

Assist in grant writing and reporting

Assist with monitoring of and modifications to eClinicalWorks to ensure quality charting and accurate data collection

Collect demographic data on acute care patients by analyzing intake paperwork

Assist Wellness Department with conducting Fitness Assessments and Goal-Setting exercises with Shalom patients

Coordinate scheduling of orthopedic referrals

Qualities and Skills:

The education requirements for this position is to have a minimum of a Bachelor’s degree in any field.

Experience is a clinical setting is a plus.

Organizational skills, interpersonal skills, and ability to be self-directed and flexible.

Ability to thrive with deadlines that require productivity, initiative and follow through.
Physical Demands: While performing the duties of the job, this role is regularly required to sit, stand, walk, speak and hear.  The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and can read a computer screen.  The employee must occasionally lift and/or move up to 20 pounds.  Reasonable accommodations may be made to enable otherwise qualified individuals with disabilities to perform the essential functions.
Work Environment: The work environment is a small, busy office located in Amarillo, Texas. The noise level in the work environment is usually low to moderate.
